Now Taking Vaccine Requests

SMC is now taking vaccine requests for individuals over the age of 18 who have severe health conditions as listed by public health. The conditions are listed below. When you call to be placed on our appointment list you must tell us what severe condition you are diagnosed with. We will only return your call when we have an appointment available for you.

  • Cancer (current diagnosis)

  • Chronic Kidney Disease (stage 4 or above)

  • Chronic Pulmonary Disease (oxygen dependent)

  • Down Syndrome Solid Organ Transplant (kidney, liver, pancreas, heart or lung)

  • Pregnancy Sickle Cell Disease Heart Conditions (heart failure, coronary artery disease or cardiomyopathies – this does not include hypertension, high blood pressure)

  • Sever Obesity Type 2 Diabetes

We are still vaccinating those 65+, educators, childcare workers, non-medical emergency services, and agriculture and food workers in addition to the severe health issues listed above.
